These are optional forms you can print out ahead of time or fill out at the office. These forms are handy for people wishing to advocate for themselves and where they are today with their emotional, physical and sensory needs.
Step by step- First in-person session
In order to help decrease anxiety about coming to the office for the first time, we have created a step by step explanation on how to access our office.
This is the inside entrance for our suite. The door should be open but if it is not.
1: See if it is unlocked
2: If locked, press the Ring Doorbell (not shown in this pic).
3: If no answer, text/email your therapist & confirm you have the right day/time. They most likely ran a quick errand and will be back soon. Go to Q&A for more info.

Please wait for your therapist to come get you. Take a seat in our lobby. The fridge has bottled water & sparkling water free to everyone as well as creamer for coffee. Help yourself to espresso or tea and we have snacks in a bowl you are able to help yourself to.
